1. How do I notify ASE if I change my address?

There are several ways to do so, depending on your preference, all of which you will need to include your name, ASE ID or last four digits of your SSN, and your old and new addresses:

  • Update your address online through myASE.
  • Submit your change of address online to ASE Customer Service by clicking here
  • By mail, submit your change of address to: ASE Customer Service, 101 Blue Seal Dr., S.E., Suite 101, Leesburg, VA  20175
  • Fax your change of address to (703) 669-6122.
  • Call ASE Customer Service at 1-800-390-6789 between 8 a.m. and 5 p.m. Eastern Time, Monday through Friday.

2. I legally changed my name. How do I get my ASE certifications to reflect my new name?

To change the name on your credentials and ASE record, please submit a copy of the legal paperwork granting the name change (i.e. official court documents, marriage certificate, etc.) along with the Name, SSN, or DOB Update Request Form. There is no charge for updating your name in our records or status letters that show your new name and current status.

However, if you want a replacement display certificate with your new name, they cost $7.50 per test series. You can get the “Duplicate ASE Credentials Request Form” from the Download Forms page.

3. I originally used a nickname. Do I need to change it to my legal name as it is stated on my driver’s license?

Yes. The name on your ASE record as shown on the Registration Ticket sent to you by email when you schedule an appointment must match the name on your photo ID.

To change the name on your ASE record, please submit a copy of your current driver’s license, military ID card, or other government-issued form that shows your full name and date of birth along with the “Name, SSN, or DOB Update Request Form” from the Download Forms page. There is no charge for updating your name in our records or status letters that show your legal name and current status.

4. I noticed that my date of birth or the last four digits of my SSN are wrong on my ASE record. How do I correct it?

To correct your DOB, please submit a copy of your current driver’s license, birth certificate, military ID card, or other government-issued form that shows your full name and date of birth along with the “Name, SSN, or DOB Update Request Form” from the Download Forms page.

To correct the last four digits of your SSN, please submit a copy of your social security card, payroll stub, military ID card, or other government-issued form that shows your full name and the last four digits of your social security number along with the “Name, SSN, or DOB Update Request Form” from the Download Forms page. There is no charge for updating your DOB or the last four digits of your SSN in our records. Note that we only need to see the last four digits. All other SSN digits can be blacked out on the copy.
